In NW, it comes from multiple accounts. In Kyn's case, he was lucky that he deleted his other accounts beforehand, otherwise he would have been smote and his account would have been locked.
People have also been known to request smitings from jorm personally, if I'm not mistaken and I believe people have been smote for attacking his characters. It's quite a spectacle, really.
