In his captor's confusion, and with his bonds broken, Alex is able to crawl to a nearby manhole and climb in. His legs screaming in agony, he threads his arms through the rungs in the ladder and holds on tight. He also checks the load on his pistol, ready to shoot himself if the situation gets hopeless.
